How can I obtain a copy of the Medical Report related to the time I was transported by the ambulance?

To obtain a copy of a medical report, simply fill the EMS Patient Care Report Authorization Request Form provided on the DFR homepage. If patient is a minor, must include a Birth Certificate. All HIPAA laws will be obeyed and enforced. There is a $15.00 charge for all EMS Patient Care Reports; however individuals and governmental entities are exempt. All authenticated and certified affidavits or patient care reports will have a $6.00 charge for each item notarized. Payment may be by check or money order only. Please make checks and money orders payable to City of Dallas Fire-Rescue Department.

Are all of the ambulances staffed with Paramedics?

Dallas Fire-Rescue Department ambulances are staffed with two Texas Department of State Health Services certified or licensed Paramedics at all times.

Why does the Fire Department send only a Fire Engine on some minor medical calls?

All of the Dallas Fire-Rescue Department Fire Engines are Advance Life Support (ALS) capable. This means that the Fire Engine is staffed with a minimum of one paramedic at all times. Also, the ALS Engines carries all of the same medical equipment that an ambulance carries. The only difference is that the ALS Engine cannot transport a patient. If the assessment by the engine's paramedics determines that a transport is necessary, an ambulance is dispatched immediately while the patient is being treated and prepared for transport. Please remember that this procedure is used only in the case of minor medical calls. Major medical calls like a heart attack, stroke, gunshot wound and others will have an ambulance dispatched immediately in addition to the ALS Engine.

Can I fill out a request and wait for my report?

If you are the patient, you may pick up your patient record in person at 1551 Baylor Street, Suite 300, Dallas, Texas 75226. The expected wait time is about 30 minutes, if all documentation is provided. If you are the patient and you would like to receive a copy of your medical records, you must complete the City of Dallas Fire-Rescue's Authorization to Use or Disclose Protected Health Information form. You can find a copy of this form by viewing our website www.dallasfirerescue.com and search/download under HIPAA Forms. If you do not have internet access, you can also request a copy of these forms by faxing your request to 214-670-5100 or calling 214-670-4311 to speak to a customer service representative. All other requestors will have to either return at a later date to pick up the record, if approval is given to release the medical record or have the report mailed to their address as listed on the authorization form.

Requestors must also present proper picture identification consisting of a driver's license, official identification in order to receive a copy of your medical record. There will be a $15.00 records retrieval fee for each complete copy of medical records. Individuals and governmental entities are exempt from this fee. Because of HIPAA rules, we are not able to fax medical records. We will mail medical records to the mailing address listed on the authorization form.

How can I get a copy of an EMS patient report?

To obtain a copy of an EMS patient report, the actual patient or another person with a medical authorization signed by the patient, along with proper ID, must request the report through the Dallas Fire-Rescue Emergency Medical Services Administration office. Due to medical information on the report we can only release these reports with the patient's consent. These reports are available at Dallas Fire-Rescue Emergency Medical Services Administration, 1551 Baylor Street, Suite #300, Monday through Friday between 8:00 a.m. and 5:00 p.m.

Can I get my blood pressure checked at the fire station?

Yes. Blood pressure checks are offered any time of the day at every fire station. Remember, in case of any illness or emergency, please call 911. Please remember that if you have any medical complaints beyond a routine medical check, additional assessment and charges may apply.

How can I relay a compliment concerning the paramedics?

For compliments concerning the Dallas Fire-Rescue Department, please contact the Fire Administration office at 214-670-6920. Or you can visit the Fire Administration office at 1551 Baylor Street, #300 Dallas, Texas 75226. The office is open Monday through Friday between 8:00 a.m. and 5:00 p.m. You can also email your information to 780@dallascityhall.com.
